Ceramides — How to Rebuild Your Skin Barrier Step by Step

Ceramides (NP, AP, EOP) are key to a healthy skin barrier. Learn their types, the ideal ratio with cholesterol and fatty acids, and CeraVe-style formulas.

What are ceramides and why are they so important?

Ceramides are lipids (fats) that make up roughly 50% of the lipid mass of the stratum corneum — the outermost barrier that protects the body from water loss and from substances penetrating from outside. Together with cholesterol (25%) and free fatty acids (10–15%), they form an organised lamellar structure — microscopic "layers" between corneocytes (the dead cells of the epidermis) that act like mortar between bricks.

When ceramide levels drop — due to ageing, over-cleansing, skin conditions (atopic dermatitis, psoriasis) or weather — the barrier becomes leaky. The result: transepidermal water loss (TEWL) rises, and skin becomes dry, irritated and prone to inflammation.

Types of ceramides — not all are created equal

Human skin contains 12 known classes of ceramides, identified by combinations of letters that indicate their chemical structure. On INCI lists, the ones you'll see most often are:

  • Ceramide NP (formerly Ceramide 3) — the most abundant ceramide in skin, responsible for maintaining the lamellar structure. It's the "workhorse" of cosmetic ceramides.
  • Ceramide AP (formerly Ceramide 6-II) — key to the proper shedding of corneocytes. A deficiency is linked to a psoriasis-like build-up of dead skin cells.
  • Ceramide EOP (formerly Ceramide 1) — a long-chain ceramide that forms "bridges" between the lamellar layers. Crucial for barrier integrity, but the hardest to synthesise.
  • Ceramide NS (formerly Ceramide 2) — the second most abundant ceramide. Important for the flexibility of the lipid layer.

The 3:1:1 ratio — the key to effectiveness

The most important discovery in lipid dermatology: the effectiveness of ceramide products depends not so much on the amount of ceramides as on the correct ratio of three lipid classes:

  • Ceramides : cholesterol : free fatty acids = 3:1:1 (molar ratio)

Research by Dr Peter Elias at the University of California showed that products containing all three lipid classes in a similar ratio repair the skin barrier faster and more durably than products with ceramides alone. What's more, a formula dominated by a single lipid (e.g. cholesterol only or ceramides only) can paradoxically slow down barrier repair.

That's why the most effective ceramide creams also contain cholesterol and fatty acids (such as linoleic, stearic and palmitic acid). On the INCI list, look for: Cholesterol, Linoleic Acid, Stearic Acid, Palmitic Acid alongside the ceramides.

CeraVe-style formulas — why do they work?

CeraVe became an iconic "ceramide" brand not because it contains a lot of ceramides, but because it uses MVE (MultiVesicular Emulsion) technology — ceramides, cholesterol and fatty acids are enclosed in multilayered vesicles that release the lipids gradually over 24 hours.

A typical CeraVe formula contains three ceramides (NP, AP, EOP) plus cholesterol — which mirrors the natural composition of skin lipids. It's not the only brand following this philosophy — the Polish market offers alternatives that also provide complete ceramide formulas.

The key question when choosing a product: does the cream contain ceramides + cholesterol + fatty acids? If the INCI list shows only Ceramide NP, with no cholesterol or fatty acids, the formula is incomplete.

When are ceramides especially needed?

Ceramides are essential for every skin type, but supplementing them is critical in the following situations:

  1. Atopic dermatitis (eczema) — studies show reduced ceramide levels (especially EOP and NP) in atopic skin. Ceramide creams are recommended as a supportive therapy.
  2. After intensive treatments — chemical peels, lasers, retinoid therapy — all of these disrupt the skin barrier. Ceramides speed up regeneration.
  3. Mature skin — after the age of 40, ceramide levels drop by 30–40% compared with young skin.
  4. Winter and indoor heating — low humidity and dry heating increase TEWL. Ceramides help to seal the barrier.
  5. Over-cleansing — harsh gels with SLS/SLES wash lipids out of the stratum corneum. Ceramides help to rebuild them.

How to spot ceramides in cosmetics?

What to look out for when analysing the ingredients of a ceramide product:

  1. At least two types of ceramides — ideally NP + AP, or NP + AP + EOP.
  2. Cholesterol on the INCI list — without it, ceramides are less effective.
  3. Fatty acids — Linoleic Acid, Stearic Acid or other free fatty acids.
  4. Position on the list — at cosmetic concentrations, ceramides are effective even at low levels (0.1–0.5%), so they may appear in the second half of the INCI list. That's normal.
  5. Phytoceramides and pseudoceramidesPhytosphingosine and Sphingolipids are ceramide precursors that the skin can convert. They make a valuable addition.
