Warming sports balms are among the most widely used products in the world of sport. Professional athletes reach for them before competition, and amateurs apply them after an intense workout. But do you actually know why your skin feels warm after applying them? It's a fascinating story about receptors, neurotransmitters and the clever way active ingredients trick the brain.

Capsaicin - Real Warmth Through the TRPV1 Receptor

Capsaicin is an alkaloid found naturally in chili peppers. Used in sports balms at a concentration of 0.025-0.075%, it is one of the most effective warming ingredients available.

Its mechanism of action is remarkably precise: capsaicin activates the TRPV1 receptor (Transient Receptor Potential Vanilloid 1) - the very same receptor that normally responds to temperatures above 43°C. In other words, capsaicin "tricks" the brain into thinking the skin is heating up, even though the physical temperature barely changes.

Here's what happens at the cellular level:

  1. Capsaicin binds to TRPV1 in the nerve endings
  2. Ion channels open for calcium and sodium
  3. The neuron sends a pain-and-heat signal to the brain
  4. Local blood vessels dilate (the visible redness)
  5. With repeated use, desensitisation occurs - a reduced sensation of pain

That final point is crucial: used regularly, capsaicin creams can genuinely reduce muscle pain by depleting substance P (the neuropeptide that transmits pain signals).

Menthol - The Cooling That Warms (the TRPM8 Receptor)

The paradox of menthol is that it produces a cooling sensation while also boosting blood flow. Menthol activates the TRPM8 receptor (Transient Receptor Potential Melastatin 8) - the cold receptor, which normally responds to temperatures below 26°C.

In sports balms, menthol plays a dual role:

  • A cooling effect - instant relief on application (concentration of 1-5%)
  • A warming effect - after a few minutes, as the stimulated nerve endings trigger a reflexive dilation of the blood vessels
  • A pain-relieving effect - acting as a mild anaesthetic (gate control theory)

Many modern balms combine menthol with capsaicin in an "ice-heat" formula - first you feel the cold, then the warmth. That's not marketing; it's the sequential activation of two different receptors.

Methyl Salicylate - An NSAID-Like Action

Methyl salicylate is an ester of salicylic acid that occurs naturally in wintergreen leaves and birch bark. Making up 10-30% of a sports balm's formula, it works quite differently from capsaicin or menthol.

Once applied to the skin, methyl salicylate:

  • Penetrates through the epidermis into the deeper tissues
  • Is hydrolysed into salicylic acid
  • Inhibits the COX-1 and COX-2 enzymes (just like aspirin)
  • Reduces the synthesis of prostaglandins responsible for pain and inflammation

Camphor - A Classic With Centuries of Tradition

Camphor is a terpenoid obtained from the camphor tree or synthesised from pinene. Used in sports balms at a concentration of 3-11%, it combines a warming action with mild anaesthesia.

Camphor's mechanism of action includes:

  • Activating the TRPV1 and TRPV3 receptors (heat)
  • Stimulating the nerve endings - a tingling sensation
  • Local pain relief by stimulating the cold receptors
  • Improving microcirculation in the tissues

Camphor is a core ingredient in many traditional preparations, such as Tiger Balm and Mentholatum.

Diclofenac Gel - The Line Between Cosmetic and Medicine

On the boundary between sports balms and medicines sits diclofenac gel (e.g. Voltaren, available on prescription at concentrations of 2-5%). It's a potent NSAID (non-steroidal anti-inflammatory drug) applied topically.

Diclofenac differs from "cosmetic" warming ingredients in several ways:

  • It has a proven clinical effect in treating musculoskeletal pain
  • It penetrates deeper into the tissues than methyl salicylate
  • It requires a prescription at higher concentrations
  • It carries potential side effects (skin reactions, and rarely systemic ones)

If muscle pain doesn't ease after 3-5 days of using OTC balms, consult a doctor rather than increasing the dose on your own.

Safety - The Rules You Must Never Break

Warming balms are generally safe, but they call for a few critical rules:

  • NEVER combine them with a heating pad or hot water bottle - there's a risk of chemical burns. Capsaicin plus external heat lowers your pain threshold dangerously
  • Wash your hands after applying - capsaicin in the eyes is an extremely painful experience
  • Don't apply to broken skin - wounds, abrasions, fresh tattoos
  • Don't wrap in cling film - occlusion multiplies absorption and the risk of burns
  • Don't use before a sauna or a hot shower
  • Do a patch test - apply a small amount to your wrist and wait 30 minutes

The right choice depends on your goal:

  • Warming up before training → menthol + camphor (fast-acting)
  • Muscle pain after training → capsaicin + methyl salicylate (anti-inflammatory)
