How do we rate toothpastes?

Toothpaste sits at the intersection of cosmetics and health products — though formally it is classified as a cosmetic and falls under EU Regulation 1223/2009. The key assessment criteria are: the concentration and form of fluoride, the type of abrasives, the presence of active therapeutic ingredients, and the absence of substances that could potentially irritate the mucous membrane of the mouth. Fluoride — the foundation of an effective paste

Fluoride is one of the most thoroughly researched ingredients in oral health protection. Its mechanism of action is well understood: the fluorapatite that forms in tooth enamel is more resistant to the acids produced by cavity-causing bacteria than natural hydroxyapatite. In toothpastes, fluoride appears in several forms:

  • Sodium Fluoride (NaF) — the most common and well-researched form, standard in pastes for adults
  • Sodium Monofluorophosphate (SMFP) — releases fluoride more slowly, used in pastes for children
  • Amine Fluoride (Olaflur) — used in specialist pastes, particularly effective on smooth tooth surfaces
  • Stannous Fluoride (SnF2) — has additional antibacterial properties, used in pastes for sensitive teeth and gums

The standard concentration for adults is 1000-1500 ppm F. Children under 6 should use pastes of 500-1000 ppm under adult supervision. It is worth consulting a dentist on which paste to choose for a child.

Abrasives — what's polishing your teeth?

Abrasives are responsible for removing plaque. Their type and concentration matter for tooth sensitivity and enamel erosion. The measure used is the RDA (Relative Dentin Abrasivity) index:

  • RDA up to 70 — gentle pastes, for sensitive enamel
  • RDA 70-100 — standard, safe for everyday use
  • RDA above 150 — whitening pastes, not recommended for daily use

The most common abrasives are Calcium Carbonate, Hydrated Silica and Dicalcium Phosphate. Pastes with an RDA above 200 should be avoided if you have sensitive gums or gum recession.

Active ingredients in specialist pastes

Beyond fluoride, the best pastes of 2026 contain active ingredients targeted at specific needs:

  • Potassium Nitrate / Arginine — block the dentinal tubules and reduce sensitivity
  • Zinc Citrate / Triclosan — antibacterial action (although triclosan is used less and less due to environmental concerns)
  • Hydroxyapatite — remineralises enamel, with some studies showing effectiveness comparable to fluoride
  • Xylitol — an inhibitor of Streptococcus mutans bacteria; it does not replace fluoride but complements it well

What to avoid in toothpaste?

  • SLS (Sodium Lauryl Sulfate) — can irritate the mucous membrane and aggravate aphthous (canker) sores. SLS-free pastes are a better choice for sensitive mouths
  • Alcohol — in pastes and mouthwashes it can dry out and irritate the mouth
  • Plastic microbeads — banned in the EU in rinse-off products, but it is worth checking whether the manufacturer uses substitutes

Whitening pastes — do they really work?

Whitening pastes work mainly mechanically (through abrasion) or by chelation (removing deposits by binding ions). Over-the-counter products in the EU cannot contain more than 0.1% hydrogen peroxide — too little to noticeably lighten teeth. Visible whitening requires a dental procedure. Do not use aggressive whitening pastes every day, and discuss it with your dentist first.

Fluoride-free pastes do not protect against cavities to the same degree as those with fluoride. Hydroxyapatite can be a supplement, but global dental organisations (the WHO and the Polish Dental Society (PTSt)) recommend fluoride pastes as the standard. Discuss the decision to go fluoride-free with your dentist.

You don't need to change your paste often if it suits your needs. A change is advisable when your oral health situation changes (e.g. sensitivity, pregnancy, braces) or on the recommendation of your dentist.

High-RDA pastes (above 150) used daily can accelerate enamel erosion, especially on already sensitive teeth. Use them no more than 2-3 times a week and avoid them after eating acidic foods.

Children under 2 — fluoride-free or a baby paste. Ages 2-6 — 500-1000 ppm F, a pea-sized amount. Ages 6-12 — 1000-1450 ppm F, a rice-grain-sized amount. Over 12 — an adult paste. Always under parental supervision until around age 7-8.
