Mandelic Acid — The Gentlest of the AHA Acids. Why Do Dermatologists Recommend It for Sensitive Skin?

On the INCI list it appears as Mandelic Acid — an alpha-hydroxy acid derived from bitter almonds that, in recent years, has earned the status of a "gentle revolutionary" in dermatology clinics. What makes dermatologists reach for it even in patients with rosacea, when glycolic acid would trigger flushing and stinging in those same people?

Molecule size — the key to understanding its gentleness

Mandelic acid has a molecular weight of 152.15 Da (daltons). For comparison: glycolic acid weighs just 76.05 Da, and lactic acid — 90.08 Da. This seemingly dry figure carries fundamental clinical significance. A larger molecule penetrates the epidermis more slowly, which means a more even distribution of action and a lower risk of localized irritation.

A study published in the Indian Journal of Dermatology (2013) showed that a peel with 40% mandelic acid produced effects in reducing pigmentation comparable to peels with 30% glycolic acid, but with significantly fewer adverse effects — including a lower rate of irritation and post-inflammatory hyperpigmentation (PIH).

Optimal pH and concentration — when does the acid really work?

In accordance with Regulation (EC) No 1223/2009 on cosmetic products, AHA acids in home-use products should have a pH no lower than 3.5. In practice, effective mandelic acid formulas work in the pH 3.0–4.0 range — while in-clinic preparations can go lower.

Concentrations fall into three categories:

  • 5–10% — everyday home care; ideal as a starting point for those who have never used acids.
  • 20–30% — superficial peels in a cosmetology clinic.
  • 40–50% — professional dermatological peels, requiring neutralization.

It's worth remembering that concentration alone doesn't determine effectiveness. A formula with pH 5.0 and 10% mandelic acid will be practically inactive as an exfoliant, because at a higher pH most molecules remain in their dissociated (ionized) form, which does not penetrate the stratum corneum barrier.

A unique trait: amphiphilicity

Mandelic acid is the only AHA with amphiphilic properties — it dissolves both in water and partly in lipids. This makes it a kind of bridge between the AHA acids (hydrophilic) and BHA, namely salicylic acid (lipophilic). Thanks to this it penetrates not only the aqueous layer of the epidermis but also reaches the openings of the sebaceous glands, which explains its effectiveness in reducing blackheads.

Inhibiting melanogenesis — more than just exfoliation

Few people realize that mandelic acid acts directly on the melanin synthesis pathway. In vitro studies (Zheng et al., 2015) showed that it inhibits the activity of the enzyme tyrosinase — a key catalyst in the process of melanogenesis. In practice this means that regular use of mandelic acid preparations can reduce sun-induced and post-acne pigmentation without the risk of depigmentation that comes with, for example, hydroquinone.

The ideal skin for mandelic acid

This ingredient is especially recommended for people who:

  • Suffer from rosacea — type I and II, where other AHA acids cause flare-ups.
  • Have darker skin (Fitzpatrick phototype III–VI), prone to post-inflammatory hyperpigmentation.
  • Are using AHA acids for the first time and need a "training" acid.
  • Struggle with melasma-type pigmentation resistant to standard therapy.

How to introduce mandelic acid into your routine?

Start with a preparation at a 5% concentration and a pH below 4.0 — apply it in the evening, twice a week. After two weeks without irritation, increase to every other evening. Don't combine it in the same application with retinoids or vitamin C in the form of L-ascorbic acid (too low a pH combined with an acidic AHA risks damaging the skin's barrier).

Yes, but with mandatory SPF 50 protection. Thanks to its slower penetration, mandelic acid is safer in summer than glycolic, yet it still increases the skin's photosensitivity. Use it in the evening and apply sunscreen in the morning.

Salicylic acid (BHA) is fully lipophilic and cleanses pores from within more effectively. Mandelic acid is amphiphilic — it works both on the surface of the epidermis and partly inside the pores, but it additionally inhibits melanogenesis, which helps reduce post-acne marks.

It is one of the few AHA acids recommended for type I and II rosacea. Its large molecule (152 Da) minimizes irritation and does not trigger the sudden flushing typical of glycolic acid.

The optimal pH is 3.0–4.0. Preparations with a pH above 4.5 have significantly reduced exfoliating ability, because the acid shifts into its dissociated form, which does not penetrate the stratum corneum.
